In order to participate in a sport, you must complete the registration and clearance process. You can find directions for how to complete this process on the Home page or by selecting the 'Clearance' page from the left hand column. Below is a copy of the Blank Physical Form  you should bring to your doctor when completing a physical examination. You will upload this document when registering your student-athlete.
