If your student-athlete has submit an academic waiver form due to potential ineligibility, they must complete and turn in this Academic Progress Report to the AD's office by Friday at 3pm. Failure to complete or submit may result in the student-athlete's ineligibility to participate in their sport. 

This form must be completed, submitted, and approved by Coach Eshelman in advance of the team's departure on the bus.
